On line 16, the value for the month field is "Jan", the suggested correction is "Jun". Submitted by Gail Gauthier. The following comment was attached: Possibly this.....there is a stroke or letter to the right of the downloop of y. However, the letter following the J appears more like an a. Your call!
This correction was evaluated by Krystal Tait: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
