On line 15, the value for the surname field is "Suckichi", the suggested correction is "Suikichi". Submitted by Karyn Snelling.
This correction was evaluated by Mark Gallop: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
