On line 31, the value for the givens field is "Zlohmas(?)", the suggested correction is "Johma(?)". Submitted anonymously.
This correction was evaluated by Jonathan Hamilton: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct. This comment was added: The first letter is not written in the same way as the 'J' in Joseph on line 29.
