On line 16, the value for the givens field is "Archie", the suggested correction is "Archi'l". Submitted by Margaret Cox. The following comment was attached: It looks like an abbreviation of "Archibald".
This correction was evaluated by Harold Hails: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
