On line 9, the value for the givens field is "Le?ie", the suggested correction is "Lexie". Submitted by Margaret Cox.
This correction was evaluated by Gerald Peskett: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
